Serves patients between 100%-200% Federal Poverty Level who have at least one working person in the family.

Spanish speaking staff and volunteers.

Monday 9am-12pm; 1:30-5pm; 5:30-8:00pm
Tuesday 1:30-9pm
Wednesday Mental Health and Diabetic patients only
Thursday 9am-12pm; 1:30-5:00pm
Friday 9am-1pm

Appointments required. Occasionally same day appointments for established patients only. Walk-in accepted on limited basis for established patients only.

Bus routes: 43, 41, 30, 36A

Services: Primary care; adults only. Specialty care services available as needed. Diabetes management and mental health groups. Prescription assistance available. Serves Lane County residents only.

Cost: Free; donations accepted. No Oregon Health Plan or insurance accepted. Must be between 100% - 200% of the federal poverty level.

If I had to start my collaborative all over again, I would start it in the way that Lane County did. Having the right people at the table and a respected community convener like United Way are the ingredients for long-term success.
